NEW PATIENT REQUEST FORM

New patient appointments are only scheduled at 9 am and 2 pm.


The information below is required to be scheduled as a new patient.


If there is more than one person needing to be scheduled, a form must be filled out for each person.

REFILL REQUESTS

!!.Ms. Jones provides for hundreds of patients, so there is an allotted time that has to pass before she can fulfill your request.!!

 

  • Refill requests are offered to primary care patients, but not urgent care patients.

 

  • If you are a primary care patient that is needing a refill, allow up to 72 hours after your initial request for a refill to follow-up on your request.

  • Refill requests should be called in 3-4 days before you run out of medication

  • If your medication is not at the pharmacy after 72 hours, call the clinic to inform the staff.

  • Urgent Care patients in need of refill must call and schedule an appointment to come in. Urgent Care refill requests without a clinic visit will not be fulfilled.

 

REFILL REQUEST FORM

Refill requests MUST be submitted 3-4 days before you run out of medication. Do not wait until you are out, as it will not speed up the refill process time.

Allow up to 72 hours for your request to be fulfilled. Multiple submitted requests or an influx of phone calls to the refill request line will result in a delayed response.

Patient Referrals

Patient referrals are an important part of patient care. Referral requests are handled by appointment only. If a referral is needed, an appointment has to be scheduled. 

 

Based on where you are referred, scans/imaging or labs may need to be done. If this is the case, we ask that you do your part so that you can be seen by the referred specialist.

If a referral was sent for you that you need to check on, call our referral line: (539) 424-5943. Please give 7 days after your initial appointment to call and check on your referral, as referrals take 7-10 days to be processed.
